Tirzepatide and semaglutide are now sold by dozens of online programs, and the price on the home page is rarely what you pay by month four. The biggest names charge a monthly membership fee on top of the medication, and the medication itself gets more expensive as your dose goes up. Others quote an introductory month that doubles at renewal. We went through six of the most searched telehealth programs and wrote down what each one actually charges, what it includes, and what happens to the price as treatment continues.
Biotech Lifestyle is our pick for most people, and we explain exactly why below the tables. Three of the six programs here bill a $149 per month membership before you have paid a cent for medication. Biotech Lifestyle charges no membership fee, one price at every dose, and a first month of $99 for semaglutide or $169 for tirzepatide.
How we compared them
Every figure below was read on the provider's own website on October 1, 2026. We did not use coupon codes, affiliate rates or third-party summaries. Where a site does not publish a number, the cell says so rather than guessing. Prices change often, so treat this as a snapshot and confirm on the provider's site before you buy.
- Price What the medication costs per month, the first-month rate if there is one, and the real total once any membership is added.
- Membership fees Anything charged on top of the medication: membership, program, enrollment or consult fees.
- Price increases Whether the price rises when your dose goes up or when an introductory month ends.
- Features Brand-name or compounded, provider access, supplies, shipping speed, coaching, HSA and FSA.

The $448 totals add the $149 membership to the lowest-dose medication price. Biotech Lifestyle prices are the live prices on biotechlifestyle.com; the quarterly plan is paid once and covers 90 days.
Semaglutide: six programs compared
Semaglutide is the GLP-1 agonist behind Ozempic and Wegovy. The same split applies: the three membership programs sell brand-name Wegovy with the fee on top, while Biotech Lifestyle, Noom Med and Shed prescribe compounded semaglutide at an all-in price.

Why Biotech Lifestyle is our pick
Four things that decide your real cost, and where Biotech Lifestyle lands on each
- No membership fee, ever Hims & Hers, Ro and LifeMD each bill $149 a month before the medication. At Biotech Lifestyle the price is the medication, the provider visit (a $49.99 value, waived) and shipping. Nothing is billed on top.
- One price at every dose Whatever vial strength your provider prescribes, the price you see is the price you pay. Brand-name Zepbound and Wegovy cost more at every step up, and the membership keeps renewing alongside.
- A low first month without a twelve-month lock $99 for semaglutide, $169 for tirzepatide. The quarterly plans ($297 and $456) cover 90 days, not a year, and you can adjust or cancel anytime.
- Everything in the box, overnight Vitamin B12 in the formulation, syringes and swabs, UPS Next Day Air shipping and ongoing provider messaging are included. HSA and FSA are accepted.
Treatment is prescribed only after an evaluation by a licensed provider. Compounded medications are prepared by licensed pharmacies for individual patients and are not FDA-approved.
If you specifically want the FDA-approved brand-name pen and your insurance covers it, Hims & Hers, Ro and LifeMD can help you get there, and LifeMD will check your coverage for you. Paying cash, though, the membership changes the math: $149 a month is $1,788 a year before any medication. If you want behavior coaching built into the program and do not mind the price stepping up after the first month, Noom Med is the most complete app. Shed is the closest competitor on price and is also flat by dose, but its best rate needs a twelve-month plan. For everyone else, a flat price with no fees and no lock-in is the simplest way to know what the next year costs, and that is where Biotech Lifestyle stands apart.
Trustpilot and Google ratings
Review scores are a blunt tool, but they catch patterns that a pricing page will not: billing surprises, shipping delays, and how a company behaves when you cancel. These are the scores shown on each provider's Trustpilot profile and, where one exists, a public Google rating, as of October 1, 2026. Each score links to the source so you can read the recent reviews yourself.
Scores change daily and are shown without review counts. Read recent reviews on the platform itself before you decide.
Before you sign up anywhere
Whichever program you choose, five questions will save you money and frustration. Every one of them is answered in the tables above for the six programs we compared.
- Is there a membership fee, and is it billed separately from the medication? A $149 membership plus a $299 pen is a $448 month, and the fee keeps billing even in months you skip a refill.
- Is the headline price the month-one price or the ongoing price? If the site says "first month" or "to get started", find the renewal number before you enter a card.
- Does the price change when your dose goes up? Most people on tirzepatide or semaglutide increase their dose at least twice in the first few months, and brand-name pens are priced by dose.
- How long are you locked in? A twelve-month prepay is a different commitment from a month-to-month plan, and most prepaid plans are non-refundable once the first shipment goes out.
- What is in the box and how fast does it arrive? Syringes, swabs and overnight or refrigerated shipping are included at some programs and not others.
Brand-name or compounded
Three programs in this comparison sell brand-name Zepbound and Wegovy, which are FDA-approved and priced by the manufacturer at each dose. The other three, including Biotech Lifestyle, prescribe compounded tirzepatide or semaglutide. Compounded medications are prepared by licensed pharmacies for individual patients and are not FDA-approved. Every program here requires an evaluation by a licensed provider before anything is prescribed, and none of them can promise a result. The right choice depends on your coverage, your budget and your provider's judgment; the tables above are meant to make the cost side of that decision clear.
